Insurance and Financial Information

As a courtesy to our patients, we will file most dental insurances for you. At each visit, we will need your insurance card for proof of coverage. Our office will estimate your insurance benefit; co-payment of the balance is expected at the time services are rendered. We accept cash, debit cards, all major credit cards, and personal checks. We require a minimum charge of $10.00 and a $25.00 fee for returned checks. We do offer financing options for patients who qualify. You may contact our office regarding this option
